The Management Information Systems Office (MIS) of Tarlac State University has devised a contactless method to answer the health survey and profile forms for visitors. This endeavor is part of the university’s compliance with government regulations particularly on COVID-19 protocols.
To accomplish the form, the university’s clients and visitors must follow these steps:
1. Using your mobile phone, scan the QR code available at the entry points/gates of the university. Click the link which will redirect you to the web-based form.
2. Fill out the health survey and profile form.
3. Present the duly accomplished and submitted form to the guard on duty.
In case the client/visitor does not have LTE/3G data, a WiFi network is available at the entrance gates.
