GENERAL INFORMATION AND GUIDELINES
- It has been 25 years since President Corazon C. Aquino signed into law Republic Act 6764 converting the Tarlac College of Technology into the Tarlac State University on October 13, 1989. From a fledgling school of arts and trades in 1906, Tarlac State University has grown into a leading institution in the region and has produced a multitude of alumni who have blazed the world and brought prestige and reputation to TSU as a producer of outstanding graduates.
- This year the Office of Alumni Affairs aims to honor its outstanding alumni by recognizing twenty five (25) of them as Distinguished Alumni in celebration of the silver anniversary of the university hood of the Tarlac State University.
- Nominations shall be made by accomplishing and submitting the nomination form to the Secretariat.
- Documentation in support of the statement of achievements, such as certified/authenticated copies (as may be applicable) of diplomas, certificates, citations, new articles, photographs, etc. may be submitted.
- Nominators may send an electronic copy of the nomination materials as an email attachment. All these must be delivered/mailed/emailed to:

6. Nominations must be received by the Search Committee not later than February 27, 2015 and/or postmarked not later than the said date.
7. The Search Committee shall deliberate on the nominations and may request additional information or documents from the nominators.
8. Supporting documents shall be subjected to the evaluation of the Search Committee. The Search Committee reserves the right to verify the authenticity of all claims.
9. The Search Committee shall select the awardees. At least 80% of the awardees are graduates of regular programs of the university. The remaining slots may come from graduates of the Transnational Education program of the university. The decision of the Search Committee is final and shall be submitted to the University President for affirmation.
10. All Distinguished Alumni Awardees must attend the awarding ceremonies on (date/time/venue to be announced) to personally receive their awards at the Alumni Center, Lucinda Campus.
11. The Search Committee shall immediately notify the awardee of his/her selection for the award. In the event that the awardee declines to accept the award, the Search Committee reserves the right to withdraw the award and qualify the next in rank.
12. All alumni awardees are requested to prepare a mini billboard (4 ft. x 8 ft.) presentation of their achievements, products, outputs, and works for display at the Alumni Center.
13. All alumni awardees are requested to submit a brief write-up (not more than 1,000 words) about their current activities and accomplishments for publication in the proposed TSU Alumni Awards coffee table book.
14. All alumni awardees must come in Filipiniana (ladies) and Barong Tagalog (men) and must be ready for solo pictorial before the awarding ceremony.
15. The nominee must satisfy all the criteria established for the TSU Distinguished Alumni Award.
16. Criteria for the Award
i. The TSU Distinguished Alumni Awards are conferred on alumni who have demonstrated, in their chosen field of endeavor, exemplary performance and outstanding contributions that bring about substantial benefits to society and distinct honor to the University. Recognition is given to achievements and contributions that impact the community in which the alumnus/alumna is/or has been active, whether this is at the local, national or international level.
ii. Graduates of the laboratory school (15 years ago and above), technical and vocational courses (10 years ago and above), baccalaureate degree programs (10years ago and above), masteral degree programs (5 years ago and above) and doctoral degree programs (5 years ago and above) may be nominated for the award.
iii. Evaluation of nominees for the Distinguished Alumni Awards will be based on the following criteria:
EXEMPLARY PERFORMANCE/LEADERSHIP IN THE FIELD/RELATED FIELD/LEADERSHIP (40%)
The nominee is a recognized expert, at least locally, in his field of specialization.
The nominee occupies a distinguished position in the organization or community where he/she belongs.
The nominee has a good track record of leadership in his/her organization/community where he works or in the professional organization where he/she is affiliated.
COMMUNITY INVOLVEMENT/SERVICE ORIENTATION (20%)
The nominee participates in community activities and shows proofs of productive participation in these activities. Involvements lead to a positive transformation of the community both qualitatively (how deep, lasting, or transformative) and quantitatively (how many people or groups benefited)
SUPPORT TO TSU (20%)
The nominee must have demonstrated a keen interest in the development of TSU through his support (past and present) for TSU programs/projects.
INTEGRITY (20%)
There is no serious concern about, or challenge to, the nominee’s personal integrity.
